EL2- Pre-Participation ExaminationEL2 Form is required to be stamped and dated by either an MD, DO, or DC on page 2. Page 1 is filled out and completed by the athlete and parent. Page 3 is if a specialist is needed for referral such as cardiologist, orthopedic, endocrinologist, etc.
McArthur Sportsmanship PolicyThe Sportsmanship Policy is signed by the parent and athlete. This form states our expectations of our Mustang athletes on and off the field/court.
Memorial Release FormMemorial Release Form
Every public school in Broward County has an Athletic Trainer from Memorial to help care for our athletes. The ATs care for our athletes on and off the court. They specialize in injury prevention, evaluation, and rehabilitation. They also specialize in musculoskeletal injuries. This form allows the AT to evaluate and treat any injury your athlete may sustain during practices or games. When our AT cannot travel to away games, the hosting school's AT will assist if needed. The ATs keep each other informed on any injuries as accurate as possible, but we encourage you to reach out to our AT, Ms. Ty, if you are injured during a sport related event so we can make sure you are clear for practices and games. We want to keep everyone healthy! Copy of Birth CertificatePhotocopy of birth certificate needed on file. If you have submitted it prior, the athletic trainer should have it on file.
GA4 for Transferring StudentsAny student who changes attendance to a member school at any time, regardless of whether the change occurs during the school year (i.e. a transfer) or during the summer period between school years, including youth exchange, international and immigrant students, or is a “Non-Traditional” student (i.e. home education, certain charter and special/alternative school, certain private school, FLVS Full Time Public Program, etc.) participating for your school. This form is not required for students entering from a terminating grade school (i.e. 5th grade to 6th, 8th grade to 9th grade).
GA4 Form EL6 for Transferring StudentsA student who changes schools after initially attending another school is subject to Bylaw 9.3 Transfer Regulations. In accordance with Bylaw 9.3.2, a student who transfers to another school during the school year may seek to immediately join an existing team if the roster for the specific interscholastic or interscholastic extracurricular activity has not reached that activity’s identified maximum size, provided the coach for the activity determines that the student has the requisite skill and ability to participate

EL3 Consent FormsEL3 form is 4 pages that must be signed by the parent/guardian and the student athlete. These are consent forms for the athlete to participate in a sport for the FHSAA.
IMPACT Consent FormEvery two years, athletes must complete the IMPACT baseline for concussion testing. Hopefully, no one acquires one, but it is a risk with athletics. This baseline allows us to work with the Joe DiMaggio Concussion Clinic, Nova Southeastern Concussion Clinic, and other clinics in Broward County. The specialists are able to use this baseline in the event your athlete is not healing as expected. It is required to be evaluated by a physician for a concussion. This baseline allows us to know what the "normal" is for each individual athlete. Please complete the consent form for allowing your child to take the baseline. They cannot compete in any competition until they have a baseline on file and 11-man football participants may not participate in any practice with helmets without the baseline.

Dragonfly Max AccountSimply create an account at dragonflymax.com and sync to McArthur's account, you will be approved to join, but can upload papers prior to approval. All athletes must have an account to keep track of injuries per the Broward County Athletic Association. Parents are encouraged to have an account to help keep communication with the athletic trainer for any injuries that may happen.
Once an account is created, you can add paperwork through your phone or scanned from your computer. Click on the athlete's "2020 - NOT READY %" and upload documents from there. Please check to make sure your paperwork has been approved, if rejected it will state why. EL14 for Non-Traditional Students This form is only to be completed if the “Non-Traditional” student wishes to participate for a public school (including a charter school) that is not the public school the student is zoned to attend pursuant to § 1002.31, F.S. Section A of this form must be completed by student’s parent/legal guardian. Section B must be completed by the public school district or charter/lab school board of the school at which the student wishes to participate (Note: this may be in a county different than the county in which the student resides). Can be uploaded in the EL-6 location in DragonFly.
